Description
FormDesk is a self-hosted form builder and submission inbox template built with Next.js, TypeScript, Prisma, PostgreSQL, Clerk, and Tailwind CSS.
Create forms, configure fields, publish public form URLs, collect validated responses, and manage submissions from one professional dashboard. The source code is suitable for developers, freelancers, agencies, and small businesses that need a customizable form workflow for their own infrastructure.
FormDesk focuses on the complete form-to-submission workflow. It does not include billing, subscriptions, checkout, AI provider integrations, or mandatory third-party services.
The package includes installation documentation, configuration guidance, security notes, troubleshooting instructions, and a clean buyer-installation test record.

Requirements
- Node.js 20 or newer
- npm
- PostgreSQL 14 or newer
- A Clerk application
- Optional Upstash Redis for shared rate limiting in multi-instance deployments
Instructions
- Extract the FormDesk ZIP package.
- Run
npm install. - Copy
.env.exampleto.env. - Configure PostgreSQL and Clerk credentials.
- Run
npm run db:generate. - Run
npm run db:migrate. - Run
npm run dev. - Open
http://localhost:3000. - Sign in, create a form, add fields, publish it, and test a public submission.
The package includes complete configuration, deployment, rate-limiting, security, and troubleshooting documentation.
External embedding, CSV export, SMTP notifications, billing, subscriptions, and AI integrations are not included in this release.
